NHS Launches Personalised Care Model

NHS England has launched its Comprehensive Model for Universal Personalised Care today. They aim to roll out the scheme to up to 2.5 million people over the next 5 years. You can read the full proposals here. There are six components to the model:
1. Shared decision making
2. Personalised care and support planning
3. Enabling choice, including legal rights to choice
4. Social prescribing and community-based support
5. Supported self-management
6. Personal health budgets and integrated personal budgets.
While we welcome the aspiration, we note that the recent report produced by the Social Policy Research Unit at York says that most of the pilot sites they have evaluated over the past 5 years are struggling to integrate support for people using PBs and PHBs at a local level. This will clearly need to be a focus during the roll-out if the ambitious targets are to be met.
